Beginning with the new school year, all River View Local School District enrollments and emergency medicals will be handled through Final Forms, an online registration platform.
If you are a NEW student to our district, please read through the following:
All parents must register their child(ren) using the link above. Once on the site, and under parent, select New Account. Follow the prompts to create your account, create your student(s), and sign your forms. Be sure to upload your documents to the Final Forms Website. **IF you have a current Preschooler, your documents are already on file and DO NOT have to upload the documents into Final Forms.
Please verify your parent account if your student is a RETURNING or EXISTING Student at River View.
Parent Registration How do I get started?
1. Check your email for an ACCOUNT CONFIRMATION EMAIL from the FinalForms Mailman. Example below.
Once received and opened, click CONFIRM YOUR ACCOUNT in the email text.
2. Create your new FinalForms password. Next, click CONFIRM ACCOUNT.
3. Your account will be confirmed and you will be logged in.
IF you do not receive an email from Final Forms, kindly check your SPAM folder. If you still cannot locate the email, please call Suzanne St. Clair at 740-824-3521. She will be able to update your email in our systems.
